I authorize Platinum Islands Express or its agents to purchase the above listed item(s) at the cost shown in the purchase order.

I authorize Platinum Islands Express to act as my agent to inspect and ship the item(s).

I am fully aware that Platinum Islands Express is acting on my behalf as a purchasing agent and is not responsible for the condition of the purchased products if delivered damaged to Platinum Islands Express.

I understand and agree it’s my responsibility to contact the supplier for compensation or replacement if the purchased products arrive at Platinum Islands Express damaged or lost.

I agree to pay all charges and fees as set out in this agreement, including any additional charges that may be incurred for customs clearance, freight charges, and taxes.
